Below is the entry form for the 2026 Ozark LMSC Short Course Yards (SCY) Championship Meet being held Saturday, March 28 and Sunday, March 29, 2026 at the St. Peters Rec-Plex, 5200 Mexico Rd, St. Peters, Missouri, 63376. 

ATTENTION: Read the Swim Meet Information and USMS Participant Waiver r.10/30/24, as you must indicate acceptance of the USMS Participant Waiver to authorize your meet entry.

Please mark if you are interested in competing in relays. Meet Registrar will contact anyone interested to set up lineups prior to meet. No relay deck entries are allowed.
